Transition Planning for Postsecondary Success: From Writing to Implementation

Effective transition plans prepare students for life beyond high school through purposeful planning, meaningful collaboration, and intentional implementation. This three-part professional learning series is designed to help educators develop student-centered, outcome-driven transition plans that support successful postsecondary outcomes. Series Includes: Session 1: In-person learning focused on age-appropriate transition assessment, measurable postsecondary goals, coordinated transition services, and annual transition goals. Session 2: Virtual coaching to reflect on implementation, problem-solve challenges, receive feedback, and strengthen transition planning practices. Session 3: In-person, project-based learning where participants apply their learning to review, refine, and strengthen transition plans using real student examples and collaborative feedback. Participants will leave with practical tools, implementation strategies, and a stronger framework for developing transition plans that connect student strengths, interests, and preferences to meaningful postsecondary goals and improved adult outcomes.
